Your personal health record creation begins with a series of simple questionnaires. Careplicity will guide you through various categories of important health information. This process is not unlike what you might experience at a new doctor’s office or during a hospital admission. The significant benefit is that this may be the last time you will have to fill out such a form. Once created, your comprehensive personal health record is always available to you, and it can be summarized, printed or be sent electronically to your health providers.
